HONOLULU — Nonprofits and schools throughout Hawai‘i will have access to $30,000 in grants through the annual AlohaCare Community Conscience Award, according to a news release.
The award was renamed three years ago as a tribute to the late Rev. Frank Chong, a community advocate and founding board member of AlohaCare. Up until his death on March 8, 2008, Chong served as AlohaCare’s Manager of Government and Community Relations.
AlohaCare will award a total of $30,000 in grants to organizations for projects that help prevent and/or treat common health issues in Hawai‘i affecting the general population; that serve the special needs of the frail elderly and disabled; or improve access to care. The deadline to submit a completed application is Monday, Oct. 1, 2012 at 4 p.m.
Criteria for the grant include:
• Organization must be a nonprofit 501(c)3 or of an equivalent tax exempt IRS status. Documentation is required. Educational institutions are eligible.
• Project must include at least one component that addresses asthma, diabetes, women’s health issues or obesity; the overall health and wellness of Hawai‘i’s aged, blind and disabled population; or improved access to health care services.
• Project must be completed within a one-year timeframe. Organization must submit a final report within three months of the conclusion of the one-year term.
• This award excludes community health centers (CHCs), as AlohaCare is currently supporting Hawai‘i’s CHCs through a variety or quality improvement program, access to care grants and other financial support.
Completed applications must be postmarked, emailed or faxed to AlohaCare by Monday, Oct. 1, 2012.
