HONOLULU — The National Transportation Safety Board says a Hawaii tour helicopter was not properly maintained when it crashed into waters near Pearl Harbor in February 2016.
KHON-TV reports the NTSB investigation shows that increased inspections by the Federal Aviation Administration of the aircraft may have uncovered the inadequate maintenance and prevented the crash.
The helicopter crash and sank while carrying the pilot and a family of four from Ontario, Canada. The crash caused injuries to the occupants and the death of 16-year-old Riley Dobson.
According to the NTSB final report, a component inside the drive shaft did not appear to be properly lubricated, which likely caused it to fail from overheating.
The report says grease was not applied as specified in the maintenance manual and other required maintenance was not completed.
