Arnold J. Hoffman
Arnold J. Hoffman, M.D., 69, of Kilauea, Kaua‘i, passed away on Feb. 5, 2004.
Born in Baltimore, Md., He received his medical degree from the University of Maryland.
His practices in Hawai‘i included Hanalei Clinic, Lihue Family Health Center, Kaiser Foundation and The Queen’s Health System at the Wailana Clinic.
He was also the Medical Director for E.M.S. for many years and founded OccuHealth Kauai when he retired from clinical practice.
He is survived by his wife Pamela, of Kilauea, father William Hoffman Sr. of Kapa‘a, brother William (Bill) Jr. of Pittsburgh, Pa., son Michael Brian of Kilauea and daughter Lisa of Saco, Maine, as well as many, many grateful patients and admiring friends.
A memorial gathering is planned for Sunday, Feb. 15, at Anini Beach Park at 3 p.m.
In lieu of flowers, the family requests donations be made in his name to the Kauai Cancer Society, 3535 Kuhio Hwy., Lihu‘e HI 96766, and to the Kauai Food Bank, 3825 Wa‘apa Rd, Lihu‘e HI 96766.

Mitsuyo U. Miyake of Waimea, Kaua‘i, died at Kauai Veterans Memorial Hospital on Feb. 7, 2004 at the age of 90.
She was a retired registered murse with ‘Ele‘ele Clinic and Waimea Dispensary.
She is survived by son, Michael T. (Phyllis) Miyake of Waimea; three grandsons, Keone (Dana) Miyake of Dublin, Calif., Bryan Miyake of Hayward, Calif., and Martin Miyake of Salt Lake City, Utah; three great-grandchildren; numerous nieces and a nephews.
Friends may visit with the family at 10 a.m. on Thursday, Feb. 19, at Hanapepe United Church of Christ. Sservice officiated by Pastor Mitchell Young will begin at 11 a.m.
Casual attire is suggested.
